What causes it
What causes a downturned mouth?
You have been told you look tired or cross when you feel neither, or you have seen a photograph where the corners of your mouth sit lower than they used to.
A mouth turns down for two reasons at once: something is pulling the corners down, and the support that used to hold them up has gone.
The pull comes from the depressor anguli oris, a muscle running from the jaw up to the corner of the mouth. Its job is to draw the corner down, and it is the muscle that makes a frown or a grimace. Over a lifetime it shortens and holds more tension, and a muscle in that state keeps pulling at rest rather than only when you use it. The muscles that lift the corner weaken over the same period, so the balance between up and down shifts.
The loss of support comes from the structures around the mouth. Fat pads in the midface shrink and descend, the bone of the upper and lower jaw loses projection, and the skin loses its hold. Without the volume that used to sit beside the corner of the mouth, there is nothing holding it level, and the pull wins.
Those two produce the line that runs from the corner of the mouth down toward the jaw, which people call a marionette line. The line is the fold where the descending tissue of the cheek meets the tissue that has stayed in place.
The complaint people arrive with is not about lines: a resting face that looks disapproving or sad to other people and does not match how they feel.
Why it happens when it does
Why do some mouths turn down earlier?
Anatomy sets the starting point. Some mouths are built with corners that sit slightly low, visible in photographs from childhood, and ageing exaggerates what was already there. A short distance between the corner of the mouth and the jaw gives the depressor less room, so its pull shows more.
Volume is the main variable after that. A fuller midface holds the corner up for longer; a lean face loses the support sooner. Significant weight loss removes it faster, and the face shows that loss before anywhere people wanted it to.
Habitual expression trains the muscle. A face that frowns, grimaces or holds tension around the mouth works the depressor more, and a muscle used more holds more tension at rest.
Sun exposure damages the collagen and elastin holding the envelope, so a face with heavy sun history sags earlier with the same volume loss.
Dental change matters here. Tooth loss, worn teeth and reduced bone in the jaw all remove support from directly behind the mouth, and the soft tissue over it drops.
Smoking adds elastin damage, reduced facial fullness and repeated pursing of the lips.
Genetics decides the frame, the fat distribution and the muscle strength, which is why this pattern runs in families and appears at very different ages.
What you can do
What can I do about a downturned mouth myself?
Home care cannot lift a corner that has lost its support, and nothing applied to the skin reaches the muscle pulling it down. What is in reach is the envelope and the habits.
Daily sunscreen protects the collagen and elastin that keep the skin holding what is underneath it, which changes how volume loss looks even though it does not prevent the loss; the American Academy of Dermatology sets out how much and how often. A topical retinoid builds collagen in the skin over months, which improves its quality and thickness.
Keep a stable weight. Repeated loss and regain costs facial volume each time, and the area beside the mouth is one of the first places it shows.
Dental health is more relevant here than anywhere else on the face. Teeth and jawbone are the support directly behind the mouth, and losing them drops everything in front.
Noticing tension around the mouth through the day reduces how much the depressor is working, though awareness alone will not lift a corner.
Stopping smoking removes three causes at once.
What does nothing: facial exercises, which work the muscle pulling the corner down; taping; and firming creams, which make a structural claim no topical product can meet. Resting your face with the lips slightly apart rather than pressed together changes what a photograph records.

Treatment
How is a downturned mouth treated?
Botox answers the muscle and filler answers the lost support, and the two are used together.
Botox placed into the depressor anguli oris relaxes the muscle pulling the corner down, which lets the muscles lifting the corner win the balance and the mouth sits more level at rest. The dose is small and placement is precise, because the surrounding muscles control the lip and the smile. The change starts within three to seven days and settles over roughly two weeks; it holds for around three to four months.
Dermal filler replaces the support that has gone. Placed deep beside the corner of the mouth and along the line running down toward the jaw, it rebuilds the volume that used to hold the corner level and softens the fold. Where the loss is broader than the mouth, filler in the midface addresses the cause rather than the symptom, since a corner falls partly because the cheek above it has emptied.
Sculptra suits general thinning across the lower face, stimulating the skin's own collagen over a series of treatments rather than adding volume on the day.
Where the skin has loosened as well, HIFU and RF skin tightening firm tissue that has lost tension.
A mouth treated with the muscle alone, on a face that has lost its support, will under-deliver. So will filler on a face whose depressor is still pulling hard.
Alongside treatment
Can creams help a downturned mouth?
Retinol 0.5%, Vivier
Builds collagen in the skin over months, improving quality and thickness. It does not lift a corner or relax the muscle pulling it down.
InterFuse Intensive Treatment LINES, skinbetter science
Supports the skin around the lines beside the mouth. It does nothing for the volume underneath them.
GrenzCine Face, Vivier
A growth factor cream supporting the skin's own repair between treatments. Skin quality, not structure.
Sheer SPF 30 Mineral, Vivier
Daily protection for the collagen and elastin holding the envelope tight around what is underneath.

Treatment at Lucie Mé
How to treat a downturned mouth at Lucie Mé in Toronto
Lucie Mé, established 2010, treats both causes at its two Toronto clinics. Botox relaxes the depressor anguli oris so the corner stops being pulled down, in a small dose placed to leave the lip and the smile working normally. Dermal fillers rebuild the support beside the corner and along the line toward the jaw, and in the midface where the loss started higher up. Sculptra suits broader thinning across the lower face, and HIFU and RF skin tightening firm tissue that has lost tension.
The Yorkville clinic at Holt Renfrew Centre serves downtown Toronto, Bloor-Yorkville, Rosedale, The Annex and the central neighbourhoods. The Fairview Mall clinic in North York serves North York, Thornhill, Vaughan, Markham, Richmond Hill and the northern suburbs. Local pages are up for both: Yorkville and North York.
Consultations are complimentary at both clinics. Book with whichever is closer.

Common questions
Questions about a downturned mouth
Can the corners of my mouth actually be lifted?
They can be levelled rather than lifted, and the distinction matters because it sets what you should expect. Two things are holding a corner down: a muscle pulling it, and missing support beneath it. Relaxing the depressor anguli oris with a small dose of Botox takes the pull off, which lets the muscles that lift the corner win the balance, and the mouth sits more level at rest. Replacing the volume beside the corner with filler rebuilds what used to hold it level. Together those two produce a visible change in a resting face, which is what people are asking about when they say their face looks unhappy. What neither does is reposition the corner the way surgery can, so a deeply downturned mouth softens rather than turning upward. The aim is a face that reads as rested rather than as treated. How much is achievable on your face depends on how much of it is muscle and how much is lost support, and that is the first thing a practitioner separates out at the consultation by watching your face at rest and in movement. Surgery is a different conversation with a different kind of result, and a consultation will say plainly when that is the honest answer to what bothers you.
Will Botox near my mouth change my smile?
Not when the dose is small and the placement is precise, and that is why this area is treated conservatively by anyone experienced with it. The depressor anguli oris sits close to the muscles that move the lip and control the smile, so product that spreads beyond the target can affect how the mouth works: a smile that sits unevenly, a lip that feels different, or difficulty with certain sounds. Those come from dose and placement rather than from treating the area at all. A small amount placed at the correct depth and distance from the corner relaxes the downward pull while leaving the smile unchanged, and what you should notice is a mouth that sits more level when your face is doing nothing. The first week can feel slightly unfamiliar while the effect develops, and it settles as it finishes developing. If a previous treatment left your smile feeling off, say so before anything is injected, since it changes the dose rather than ruling out treatment. This is also an area where less is genuinely better, and a practitioner starting low with a review afterwards is working correctly. Eating and drinking are unaffected at the dose this area takes, since the muscles that close the lips are not the target.
Is it the muscle or the volume loss causing mine?
Usually both, in a proportion that decides the treatment, and there is a check that gets you close. Look in a mirror with your face completely relaxed, then gently support the skin beside each corner of your mouth with a fingertip and lift a few millimetres. If the corner looks right with that small amount of support, missing volume is doing most of the work and filler is the main answer. If the corner still reads as pulled down even with support, the muscle is contributing more and relaxing it matters. Then watch yourself frown: a depressor that stands out strongly during a grimace is a muscle holding real tension. Most faces need something from each side, because the two causes feed each other, and a plan that treats only one is the usual reason someone comes back disappointed. A practitioner does this assessment by watching your face at rest, in a smile and in a frown, and by looking at what has happened to the cheek above the corner, since a mouth can fall because the midface has emptied rather than because of anything local. Do the check in daylight with your face at rest for a few seconds first, because a face that has just been talking is rarely as relaxed as it feels.
What does treatment for a downturned mouth cost in Toronto?
Cost depends on which cause is doing the most and how many areas are involved, so a number quoted before an assessment would be a guess. Botox for the depressor uses a small amount, priced by the units used, and it can be done alongside other areas at the same appointment, which changes the total rather than the cost of this part. Dermal filler is priced by the product used, and treating beside the corner and the line toward the jaw is a different figure from a plan that also rebuilds the midface above it. Sculptra is priced by the series. Where firming is part of the plan, HIFU and RF skin tightening are priced by area and session. Someone whose corner is held down mainly by muscle tension is looking at the cheapest version of this. The complimentary consultation is where the proportion gets worked out and the plan and cost are set out for your own face, including which part to do first. The two treatments also keep different schedules, so first-year cost and maintenance cost are different figures. Treating the muscle alone is the smallest version of this plan, and it is a reasonable way to find out how much of your own pattern is muscular before spending on volume.
Why does my face look sad or cross when I feel fine?
Because a resting face is read by other people from a few fixed signals, and the angle of the mouth corners is one of the strongest. When the corners sit below the line of the lips, the face reads as unhappy regardless of mood, and people respond to that reading rather than to how you actually feel. That is what people describe when they ask about this, and it is a reasonable thing to want changed. Two causes produce it: a muscle that pulls the corners down and holds tension at rest, and the loss of the volume that used to hold them level. The deepening line from the corner toward the jaw adds to the impression by casting a shadow. Treating the corner angle changes the resting signal, which is why what changes is how a resting face reads rather than the shape of it. Nothing about this is vanity, and describing the effect it has on how people treat you is useful information at a consultation, because it tells the practitioner what you are actually asking to change rather than which line to fill. Candid photographs show the resting signal better than a mirror does, since a mirror catches a face that is paying attention to itself.
