Thin Lips

A lip that was always small, or a lip that has lost its edge and its fullness. Lip filler treats both, and not in the same way.

What makes lips look thin?

What causes it

What makes lips look thin?

Lipstick that used to stay put now creeps into the fine lines in the skin of the upper lip. Or a photograph catches a smile with almost no pink lip in it.

A lip has a small number of parts, and thin lips can come from any of them. The vermilion is the coloured part, the pink or brown you see, and it is a modified mucous membrane three to five cells thick against the sixteen layers of ordinary facial skin. The vermilion border is the line at its edge, raised slightly by a pale ridge called the white roll, and the ridge is what holds lipstick in place. The philtrum is the shallow central groove of the upper lip, set between two ridges built from dense collagen and elastic tissue. Underneath all of it sits the orbicularis oris, the ring of muscle that closes and purses the mouth.

Thin lips covers two different situations that get lumped together under one phrase. Some people have always had a smaller or shorter upper lip, with little pink showing when the face is at rest and more gum than lip on a wide smile. A long philtrum, a vermilion that rolls inward rather than outward, or simply a narrow band of coloured lip all produce it. Nothing has changed and nothing is wrong; the proportions were always these.

Other lips used to look fuller and changed. When researchers compared upper lips from people under 40 with upper lips from people over 80, they found thinner skin, a thinner and shrunken orbicularis oris, collagen and elastic fibres broken down, and a wider muscle angle at the vermilion border. Their conclusion was that the older lip comes from lost elasticity and the drop that follows it, rather than from the total loss of volume people assume. The philtrum goes the same way, because its ridges are built from elastic tissue and flatten as that tissue weakens.

What you notice is the order those changes come in. The border stops holding a clean edge, so colour creeps off the lip. Less pink shows at rest. The groove down the centre of the upper lip fades.

Sun and smoking both bring that pattern forward. The vermilion carries very few melanocytes, which is why it is red at all and why it has little of the pigment that shields skin elsewhere, and the lower lip takes sun more directly than the upper one does. Smoking narrows the small vessels feeding the skin and raises the enzymes that break down collagen and elastin, and the pursing adds the vertical lines in the skin of the upper lip.

The two situations are not treated the same way. Treating a lip built small as though it had lost its edge, or the reverse, is how a result comes out looking wrong rather than merely small.

Why it happens when it does

Why do some lips thin earlier than others?

No age starts it. What differs from one face to the next is the lip someone began with and how fast the skin and muscle of that lip change.

The proportions are inherited: the length of the philtrum, the width of the coloured band, whether the vermilion rolls outward or inward. A short upper lip showing little pink at rest has been that shape since childhood, and a lip like that is not a lip that lost anything.

Sun acts on the part of the face with the least protection from it. The vermilion holds very few melanocytes, and the lower lip is angled to take sun more directly than the upper lip. Ultraviolet light damages the DNA of skin cells daily and leaves elastic fibres disorganised, and the collagen and elastin that hold a lip's shape go with them.

Smoking works on both halves at once. Nicotine narrows blood vessels and cuts the oxygen and nutrients reaching the cells, other chemicals in tobacco smoke raise the enzymes that degrade collagen and elastin, and the pursing folds the skin of the upper lip in the same places every time.

Condition counts for how a lip looks on any given day. A dry, flaking lip looks thinner than the same lip in good order, and some of what is sold for lips is what keeps them dry.

Which of the two situations you are in decides what helps. A lip that has always shown little pink is a different starting point from a lip whose edge has gone soft and whose band of colour has narrowed, and the two lead to different treatment even though both get called thin.

What can I do at home?

What you can do

What can I do at home?

Nothing you put on a lip adds substance to it or rebuilds a vermilion border. Home care protects the lip you have and changes how it looks in a mirror and in a photograph, and both are worth doing before you spend money on anything else.

Sun protection is the one with a lasting effect. The American Academy of Dermatology advises a non-irritating lip balm at SPF 30 or higher before you go outdoors, reapplied every two hours while you are out, and names zinc oxide and titanium oxide as the sun-protective ingredients to look for. The same page lists what to keep off a lip that is dry: camphor, eucalyptus, menthol, phenol, salicylic acid, fragrance, and flavourings such as cinnamon, citrus and mint. Castor seed oil, ceramides, dimethicone, shea butter, mineral oil and petrolatum are what it recommends instead.

Stopping smoking takes away the pursing that folds the skin of the upper lip, and the damage to collagen and elastin underneath.

Makeup does more for a small lip than any product does. Lip liner drawn on, or very slightly outside, the natural border puts back the edge that has softened, and it keeps colour on the lip. A lighter shade or a touch of gloss in the centre of the lip catches the light and makes the lip look fuller; a matte dark shade does the opposite. None of it survives a meal, and none of it changes the lip underneath.

Plumping glosses do something real and it lasts about an hour. In a clinical trial of one containing capsaicin, cinnamon and menthol, every subject was rated fuller fifteen minutes after application and two thirds still were at an hour, with the measured difference gone by two hours; the tingling and the heat come from those three ingredients. A study of a ginger-based plumper found lip blood flow raised for the first thirty minutes and the vermilion pushed out by half a millimetre at fifteen minutes, and recorded that no noticeable result was obtained from long-term use. The mechanism is blood flow rather than tissue, and two of those ingredients are on the Academy's list of what irritates a lip.

Suction devices sold as lip plumpers have no published trial behind them, so nothing supports a change that outlasts the swelling. Facial exercises train the orbicularis oris, the muscle that closes the mouth, and no published trial shows that working that muscle widens the band of colour above it.

Where the lip itself is what you want changed rather than how it photographs, that is what treatment is for.

Treatment

How are thin lips treated?

Lip filler answers most of what people mean by thin lips. It is hyaluronic acid gel, placed into the lip through a fine needle or a cannula. Hyaluronic acid is a sugar the body already makes and already holds in skin, so the gel sits with the tissue rather than apart from it, and an injection of the enzyme hyaluronidase dissolves it if the result is not what was wanted. Juvéderm is the hyaluronic acid range used here, and it comes in different densities so the product can be matched to the job.

Two separate things get treated, and a result looks wrong when the two are confused. The vermilion border is rebuilt by placing filler along the line itself, which puts back the edge and keeps lipstick on the lip. The body of the lip is treated separately, by replacing what thinned. A lip that has only lost its definition may need nothing but the border. A lip that has lost substance needs both. Used this way filler is corrective: it puts back roughly what was there rather than building something new.

The change is visible immediately, settles over about two weeks as swelling resolves and the gel integrates, and lasts six to eighteen months, toward the shorter end in the lips because the mouth moves more than anywhere else filler is placed. Published reviews of hyaluronic acid gels put the range at four to eighteen months depending on the product, the technique and the person. Full detail on how shape and volume decisions are made is on the lip enhancement page.

The overdone look people are afraid of has a specific cause and it is not the filler. It comes from ignoring the lip's own proportions: filling the body while leaving the border undefined, or adding a fixed volume regardless of the width and curve of the mouth it is going into.

The lip flip is the smaller, secondary option, and it answers a narrower question. It uses Botox rather than filler, relaxing the upper part of the orbicularis oris so the lip turns outward and shows more of the pink that was already there. Nothing is added. When seventeen women were measured before and after a standard four-unit treatment, upper lip height had risen at fifteen days and lip volume had not changed. It suits an upper lip that is short, or that rolls inward on a smile, and it does little for a lip that has lost its border, because the muscle is not what changed. Botulinum toxin weakens a muscle within two to four days, reaches its full effect at about ten days, and holds for roughly three months before the nerve endings regrow.

The two are not alternatives. They answer different questions, and they are sometimes used together, filler for the border and the body, a lip flip for the roll of the upper lip. Which applies is a judgement about one particular lip.

Common questions

Questions about thin lips

What is a lip flip, and how is it different from lip filler?

A lip flip uses Botox to relax the orbicularis oris so the upper lip turns outward and shows more of its own pink, while lip filler uses hyaluronic acid gel to put back an edge and a substance that are no longer there. The difference is position against material. In a study of seventeen women given a standard four-unit lip flip, upper lip height had increased at fifteen days and the volume of the lip had not changed at all, which is the whole mechanism in one line: the same lip, sitting differently. Filler does the opposite. It adds gel along the vermilion border to rebuild the edge, or through the body of the lip to replace what thinned, and the lip afterward holds material it did not hold before. The two suit different starting points. A lip flip helps an upper lip that is short or that rolls inward on a smile, because what is hiding the pink is the position of the muscle. Filler helps a lip whose border has softened, or a lip that was built with a narrow band of colour and needs more than a change of position. A systematic review of the lip flip found the technique settled on four to six units into the orbicularis oris, with mild and short-lived effects such as difficulty whistling or drinking through a straw. Neither is permanent, and which one fits is a judgement made by looking at one particular lip at rest and on a smile.

Will lip filler look overdone, and what causes that look?

The overdone look comes from ignoring the proportions of the lip being treated, not from the filler itself. It happens when volume is added through the body of the lip without re-establishing the vermilion border, so the lip pushes outward with no defined edge, or when a fixed amount of filler is used regardless of the width and curve of the mouth it is going into. A lip that already has good proportions and needs only its border restored looks nothing like a lip that has had volume added without regard for its own shape. Timing contributes too. Hyaluronic acid filler swells for the first few days after injection, and a photograph taken in that window shows something fuller than the settled result at two weeks. A measured, border-first approach matched to the shape of the lip is what keeps a corrected lip looking like a lip, and that is a judgement made at consultation on the specific lip involved rather than a fixed technique applied the same way to everyone. It is also why two people who both ask for a natural result can end up with genuinely different amounts of filler. Two things make the outcome less final than it looks. Hyaluronic acid can be dissolved with an injection of hyaluronidase, so an overfilled lip is not a permanent state, and a lip can be built in stages, with a smaller amount placed and looked at before any more is added.

How long does lip filler last, and what happens when it wears off?

Hyaluronic acid filler runs six to eighteen months across the face and sits toward the shorter end of that in the lips, and as the gel breaks down the lip goes back to the shape it had before. Published reviews put the range at four to eighteen months depending on the product, the technique and the person. The reason lips sit low in the range is movement: talking, eating and smiling work the orbicularis oris constantly, and the same gel placed in a cheek or along a jawline, which move far less, holds longer. Nothing about the fade is abrupt. Filler integrates with the tissue around it and breaks down gradually, so a lip softens over months rather than deflating on a particular day, and a repeat treatment booked before the previous one has fully gone keeps the look steadier than letting it wear off completely each time. The result also does not arrive all at once. The change is visible as soon as the appointment is over, lips swell more than any other area filler is placed in, and the shape you keep shows at about two weeks once the swelling has resolved and the gel has settled. If you decide you do not want it before then, hyaluronidase dissolves hyaluronic acid filler, and the effect of that enzyme is visible within a day or two. Nothing about the lip is worse for having been treated when the filler has gone.

Why does my lipstick no longer stay on my lip?

Because the vermilion border has softened, and that raised edge is what held the colour in place. Outlining the coloured lip is a pale ridge two to three millimetres wide called the white roll, formed by the orbicularis oris bulging underneath it. When researchers compared young and old upper lips under the microscope, they found the angle of that muscle at the vermilion border widened with age, alongside thinner skin and broken-down collagen and elastic fibres. The ridge flattens, the line between lip and skin blurs, and colour that used to stop at the edge travels into the fine vertical lines in the skin of the upper lip. Those lines come from the same two sources as everything else here: the orbicularis oris folding the skin in the same places every time the mouth purses, and ultraviolet light breaking down the collagen and elastin that let the skin spring back. Lip liner drawn on or very slightly outside the natural border is the answer that costs nothing, and it works for exactly as long as it stays on. Filler placed along the border itself is the treatment that answers the same thing, because it rebuilds the edge rather than drawing one. A lip in this position may need nothing placed through the body at all, and the border on its own may be the whole treatment.

What does treatment for thin lips cost in Toronto?

What it costs depends on which treatment the lip needs and how much of it, so a figure quoted before anyone has looked at your face would be a guess. The cost of a lip flip follows the number of Botox units used, which follows how much the muscle needs to relax; the published technique sits at four to six units, and it is a small treatment. The cost of filler follows how much border and how much body the lip needs. Someone correcting an edge that has gone soft is in a different position from someone replacing substance across the whole lip, and someone whose lip was built with a narrow band of colour is in a third position again. Some people have both, a lip flip for the roll of the upper lip and filler for the border, which changes the arithmetic once more. Duration belongs in that arithmetic. Botox holds for roughly three months before the nerve endings regrow, while hyaluronic acid filler in the lips runs toward the shorter end of a six to eighteen month range, so the two do not come round for repeating on the same schedule and comparing them on the price of one appointment misses what actually adds up across a year.
